DOCCS Grade Report Request

This workflow is used by DOCCS authorized users to request grade reports for students in SUNY Office of Higher Education in Prison (OHEP) supported programs.

Instructions and Guidance

  • All requests must include a FERPA-compliant student authorization and release form. This form is required to verify the student's identity and authorize the release of educational records, such as transcripts and grade reports. To comply with federal and state laws governing the privacy and security of student records, the authorization must include sufficient identifying information to allow us to validate the student's identity.

  • You may obtain the FERPA-compliant release form by downloading it from our site using this link.
     
  • Please complete all required fields below and upload the signed authorization form with your request. Providing any additional student information that is readily available may help us locate records and process your request more quickly.

Required Student Information

Please include the complete name (First Last). If a middle name or intial is known, please also include it.
Please include the student's Campus ID if available. DO NOT include a social security number or any part of a social security number.
Please provide the name of the facility or site the student is currently associated with.
Please list the student's former facility or facilities only if the student participated in the same program at those locations.

Campus and Term Details

Please select the SUNY campus from which you are requesting records.
If you need records from more than one campus, please submit a form for each campus to ensure your request is routed correctly.
Please provide the year, years or range of years the student was enrolled in the college-level program. This is used to match student records, and may be your best estimate if not provided.
If requested or required, please provide additional information needed to locate records. DO NOT include a social security number or any part of a social security number.
A signed FERPA-compliant Records Release Form is required. You may also include any official or other supporting documentation needed to identify the student or process the request.
